Ashwagandha (KSM-66): An Evidence-Based Look at the Adaptogen
Ashwagandha has exploded in popularity, and for good reason — the clinical data is actually promising. But the extract, dose, and duration all matter. Here's my pharmacist's breakdown.

What Is Ashwagandha?
Ashwagandha (Withania somnifera) is an adaptogenic herb used for over 3,000 years in Ayurvedic medicine. Its active compounds — withanolides — modulate the HPA (hypothalamic-pituitary-adrenal) axis, which governs our stress response.
Modern clinical research has validated several traditional uses, making it one of the better-evidenced herbal supplements on the market.
Evidence-Based Benefits
Stress & Cortisol Reduction: Multiple RCTs demonstrate that KSM-66 ashwagandha (600 mg/day) significantly reduces perceived stress scores and serum cortisol levels vs. placebo. The CONSORT 2010 trial showed a 27.9% reduction in cortisol.
Anxiety: A 2019 RCT in Medicine showed significant reduction in anxiety and insomnia with 240 mg/day of a standardized extract over 60 days.
Sleep Quality: Studies show improvements in sleep onset latency, total sleep time, and sleep quality — particularly in people with insomnia. Takes 4–8 weeks to see effect.
Testosterone & Male Fertility: A double-blind RCT found 600 mg/day KSM-66 increased testosterone by ~15–17% and improved sperm quality in infertile men.
Strength & Recovery: 2021 meta-analysis found ashwagandha significantly improved VO2 max, muscle strength, and recovery time in resistance-trained individuals.
Choosing the Right Extract
This matters enormously:
| Extract | Standardization | Research | |---------|----------------|----------| | KSM-66 | 5% withanolides (root-only) | Most clinical evidence | | Sensoril | 10% withanolides (root+leaf) | Good evidence, more sedating | | Generic powder | Unknown potency | Unreliable |
My recommendation: KSM-66 for most uses. Sensoril if you want more sedation for sleep.
Dosing
- Stress/anxiety/sleep: 300–600 mg KSM-66 daily (can split morning/night or take all at night)
- Athletic performance: 600 mg/day
- Duration: Minimum 8 weeks for full benefit
Pharmacist Safety Note
Generally well-tolerated. Mild GI upset in some — take with food.
